Supernumerary tooth is a rare event, which is usually found coincidentally in radiographic examination. The prevalence of this event is 1-3% with different prevalence for primary and permanent dentition. Supernumerary teeth may cause various clinical problems including: delayed or failure of eruption, displacement or rotation of permanent teeth and cyst formation. The purpose of this study was to present the case of a 9-year-old boy who had a large dentigerous cyst around his primary mandibular left premolar that was enucleated with the tooth. A year after this enucleation, we have observed a supernumerary tooth in mesial of canine. It is a reasonable question that “can the supernumerary tooth formation relate to the dentigerous cyst around impacted tooth?” Therefore, it is necessary to perform more pathological investigation in this aspect to answer this question.

Objective: This study aimed to evaluate the association between orthodontic treatment and success of alveolar cleft bone grafting performed in oral and maxillofacial surgery department of Shariati hospital at least one year after operation.Materials and Methods: The patient’s records and their post-operative radiographs were studied to assess their age, gender, cleft type (unilateral or bilateral), grafting time (secondary, tertiary), orthodontic treatment (No, Yes) and height of bony bridge. The success was defined as a proportion of at least ¾ for the remaining bone in new radiograph to post-operative bone bridge height. Results: Mean age of the patients was 18 years with a range of 10 to 29 years. From the total patients, 52% (n=28) were male and 48% (n=26) were female, 54% had unilateral and 46% had bilateral alveolar cleft. Among the patients, 20 cases (37%) had secondary and 34 had tertiary bone grafting. In general, 59% (n=32) of the patients, had successful alveolar bone grafting. Out of the total 54 patients, 28 cases (52%) were under supervision and treatment of orthodontist. Logistic regression model showed that presence of orthodontic treatment was associated with higher graft success. Conclusion: Treatment of Cleft lip and palate needs comprehensive surgical and orthodontic management and all the patients must be under supervision and treatment of orthodontists to take benefit of the surgical treatment.

Purpose: The long-term outcome and clinical results of gaparthro plasty used for the treatment of condylar ankylosis of the mandible in children with application of postoperative activator appliances and costochondral rib graft are evaluated and compared.
Materials and Methods: A nonrandomized, retrospective clinical study of 10 cases of condylar ankylosis (18 Joints) of the mandible surgically treated during a 12 year period from 1990 to 2002 was performed. Four patients were treated by condylectomy and interpositional Flap and Six patients were treated by condylectomy and Immediate costachondral rib grafts.
The First group underwent long-term postoperative therapy using removable activator appliances. Casts, radiographs, photographs, computed tomography (CT) Scans, were Used post surgically to evaluate rib graft, condylar growth and function, occlusion, and Facial and condylar symmetry.
Results: Symmetry, arch coordination, correction of occlusal canting, mandibular deviation, facial growth , and prevention of reandkylosis were obtained and better controlled in those cases that cnderwent long-tern orthodontic activator therapy post operatively and were followed closely.
Conclusions: Children with long-standing condylar ankylosis of the mandible and its resultant facial asymmetry and occlusal canting (Secondry to a non functional joint and Maxillary compensation) treated with condylectomy and interpositional flap treated more favorably when activators were used postsurgically. The patients that failed to comply with or continue activator therapy postsurgically developed complications relating to mandibular deviation , occlusal dysharmony, asymmetry and rankylosis of the temporo mandibular joint (TMJ).
